AI-Powered Connected Care

Your Partner Throughout the Health Care Journey

Healthcare networks run on many hands: administrators, partner organizations, clinics, providers, care teams and the patients they serve. Each group worked from its own spreadsheets, phone calls and portals. We built UNIZA, one secure AI-powered platform that brings every role together, keeps patient information and workflows in sync, and helps teams close care gaps before they become problems.

Impact Metrics

  • 6 Access levels (Plus custom roles)
  • 10 Data sources (Automated ingestion)
  • 20 Minute data refresh (Near real-time sync)
  • 38 Dashboard insights (Tailored to each role)
  • 0 Patient identifiers (Shared with the AI)
  • 6 yr Audit history (Tamper-evident log)

Every Role, One Platform

Each level sees only what sits beneath it. A network administrator sees the whole network; a provider sees their own patient panel; a patient sees their own record. The same boundaries apply everywhere, including the AI assistant.

  • Network Administrators Network-wide operations, reporting and oversight
  • Partner Organizations The clinics they manage
  • Clinics & Care Teams Their providers, staff and schedules
  • Providers Their own patient panel
  • Patients Their records, visits and care team

The Challenge

Data Stuck in Spreadsheets

Patient, claims, quality and authorization data arrived in periodic file exports. By the time anyone reviewed it, the information was already out of date.

Access That's Hard to Get Right

Every clinic and provider should see their own patients and no one else's. Permissions built by hand across many organizations break easily, and in healthcare one slip is a compliance incident.

Care Gaps Found Too Late

Missed screenings, open quality measures and hospital discharges without follow-up showed up in reports weeks later, after the chance to act had passed.

Conversations Outside the Record

Care teams and patients coordinated by phone, email and fax. That work was never linked to the patient record and left no audit trail.

AI-Powered Solutions

Problem: Operations teams needed quick answers, like "which patients in this clinic are overdue for a screening?", but had no way to get them without a data analyst.

Solution: A built-in AI assistant answers questions in plain language. It understands what is being asked and pulls the right data on care gaps, claims, medications, hospital stays, authorizations and appointments. Everyday questions are answered instantly, deeper analysis goes to a more capable model, and answers appear as they are written.

Problem: Sharing patient information with AI is a privacy risk most healthcare organizations won't accept.

Solution: Patient identifiers are removed before any data reaches the AI and restored only once the answer is ready. Safety guardrails are always on, every request is screened, and the assistant can only look at patients the user is already allowed to see.

Problem: Data files needed manual cleanup before anyone could use them.

Solution: An automated pipeline checks for new data every 20 minutes and brings in 10 data sources, from patient rosters to claims, quality measures, hospital stays and authorizations. It skips duplicates, catches incomplete or changed files, and securely reads clinical records sent by other healthcare systems.

Problem: Quality gaps were discovered after the reporting period closed.

Solution: AI reviews each patient for open care gaps and recommends next steps. Care teams can assign, dismiss or complete each one with notes. Admissions, discharges, authorization updates and new care gaps trigger alerts automatically, and those alerts never show patient details on a lock screen.

Problem: Each role needs a different view, and one misconfigured screen could expose another clinic's patients.

Solution: Access is enforced on the server for every request, not just hidden in the interface. Six access levels combine with custom roles and per-person permissions. A patient outside a user's care network simply doesn't exist for them, so the platform never confirms that the record is there.

Inside the Platform

Screens from the live platform. Patient names, member IDs, dates of birth and provider names have been replaced with sample data and blurred.

Where Cost and Risk Concentrate

Dashboards built around who needs attention, not around data tables. Every number opens the people behind it.

AI That Finds the Gap Before the Report Does

Care gaps, health summaries and plain-language answers, all drawn from the same live data and the same access rules.

Built for an Audit From Day One

Roles decide what each person can see, and every access is written to a log the network can hand to an auditor.

Who Needs Attention Today

The network overview opens on the members who need action: who is in hospital now, who is at risk of a 30-day readmission, who drives the highest cost, and where care gaps and authorizations are piling up.

  • Filter by today, week, month or a date range
  • Every tile opens the members behind it
  • The same view is scoped to each role's own patients

Network overview: hospital census, readmissions, high-cost members, care gaps, authorizations and Rx spend.

A Board Each Person Arranges

My Board starts from the role's overview and lets each user pick the reports that matter to them from a shared catalog. The layout is saved to their account and locked until they choose to change it.

  • 12 report widgets in the catalog
  • HEDIS Stars estimates and referrals by specialty
  • Saved per user, locked by default

My Board with estimated HEDIS Stars by measure and referrals by specialty.

Cost, Risk and Quality in One View

Below the attention tiles, the overview breaks spend down by drug and claim type, groups members into risk cohorts and tracks every HEDIS measure against its Stars tier, all from the same claims and census feeds.

  • Top drugs by paid amount, from pharmacy claims
  • Risk cohorts for polypharmacy, high cost, open gaps and readmits
  • Medical and pharmacy spend split at a glance

Cost, risk and quality: high-cost Rx, risk cohorts and total cost of care.

Authorizations in One Queue

Pending and determined authorizations sit side by side, with the procedure, decision and diagnosis on every row. Staff search by member, procedure code, diagnosis or facility instead of chasing faxes.

  • Aging queue with past-SLA requests surfaced
  • Search by member, CPT, diagnosis or facility
  • One click to the member's full profile

Pre-authorization aging queue. Member names and plan numbers blurred.

From a Number to the Members Behind It

Clicking Open Care Gaps on the dashboard lists every member with a non-compliant quality measure, their primary care provider, the measure involved and how many gaps are open.

  • HEDIS measures by member and provider
  • Report and admit dates side by side
  • Straight through to the patient profile

Open care gaps drill-down. Member and provider details blurred.

AI Care Gap Analysis on Every Profile

The patient profile reads quality measures and claims together and explains, in plain language, which measures are open, the diagnosis codes behind them and the next step for the care team. A health journey summary sits alongside it.

  • Every gap backed by ICD-10 evidence
  • A recommended next step per measure
  • Analysis cached for 24 hours, refreshed on demand

Patient profile with AI care gap analysis. Patient details blurred.

Ask the Data in Plain Language

The UNIZA assistant is docked on every screen and grounded in the network's own data. Ask which members have the most open care gaps and it answers from the records the user is already allowed to see.

  • Answers stream in as they are written
  • Patient identifiers removed before the AI sees anything
  • Every request recorded in the audit log

Six Roles, Plus Custom Ones

Every level of the network, from network administrators down to clinics, providers and patients, gets its own system role. Administrators can build custom roles and adjust permissions for a single person.

  • Six built-in roles
  • Custom roles with per-person permissions
  • Changes applied instantly

Role management with the six system roles.

Every Access, on the Record

The audit log records who touched what, how and with what result, including every AI request. It stores user IDs, routes and outcomes only, so clinical values never end up in the log.

  • Filter by date, user, record, resource or outcome
  • AI invocations logged alongside page views
  • Tamper-evident and kept for six years

Audit log of access events. User details blurred.
